Discovery Place Preschool Registration

Our registration form has three options for completing and returning: 

Option #1: Save the form to your computer, fill it out digitally and email it back as an attachment
     Instructions for filling out this form on your computer:

• a) Save the registration form to your computer. Close the download and open the file you saved to your computer. Do not​ fill out the form in your web browser. Skipping these steps or filling out the form in the web browser will result in your information not being saved. 
• b) Fill out the form. 
• c) Send the form to this email as an attachment.

Option #2: Print the form, fill it out with a pen, scan to your computer and email back ​

Option #3: Print the form, fill it out with a pen and bring it to the school
